*REMEMBER:*
 For your child to receive a vaccine, HCS must have a completed consent packet. Forms will be sent home with children before vaccines are scheduled at their school. Please complete and sign all forms and return them to your school as soon as possible after you receive them. If you have not received forms and would like to have your child vaccinated, please contact your school's main office.
